This survey is being run by the Acute Leukemia Advocates Network (ALAN), CLL Advocates Network (CLLAN) and CML Advocates Network (CMLAN).
The questionnaire is about your diagnosis, care and treatment for leukemia. Its purpose is to provide information to help us understand the key issues, experiences, and unmet needs for leukemia patients.
Who should complete the questionnaire?
This questionnaire can be answered by anyone aged 18 and above directly or indirectly affected by a diagnosis of leukemia, whether this is somebody who has received a diagnosis or who is / has been a carer to someone with a diagnosis.
Two question sets have been designed, one for patients, and one for carers.
Each of these surveys will take 20 to 30 minutes to complete and is totally voluntary.
